Warranty/Returns

All products returned to Varian Medical Systems must have an RA number clearly marked on the packaging and shipping documents!

Returning Housings for Credit:

Please complete a Housing Credit Request form to request an RA number. An RA number is required for Housing Credit.
Please use the Housing Credit Request form for Housings Credits only.
Complete the form online, or download the word version (link below) and fax to 843-760-0079 or e-mail to interay.warranty@varian.com to request an RA number.
Mark the shipping container and paperwork with the RA number.
Please ensure that all housings are cleaned of blood, contrast media, and any body fluid residues before returning.
Ship the failed housing to the address indicated on the form.
Customer assumes responsibility for freight charges and any damage that may occur during shipping.

Returning an X-Ray tube for Warranty Credit:

Please complete an X-Ray Tube Service Report to request an RA number. An RA number is required for warranty credit.
Please use the X-Ray Tube Service Report form for warranty credit requests only.
Complete the form online, or download the word version and fax to 843-760-0079 or e-mail to interay.warranty@varian.com to request an RA number.
Mark the shipping container and paperwork with the RA number.
Please ensure that all X-ray tube units are cleaned of blood, contrast media, and any body fluid residues before returning.
Ship the failed X-ray tube to the address indicated on the form.

A note about credits issued for warranty:

If Varian's warranty evaluation process determines that a tube failure is covered by our warranty a credit is applied to our customer's account. The customer can apply that credit to open invoices, future orders, or receive a payment of the funds. To replace a failed X-ray tube customers must send us a new purchase order.

Housing exchange: ( Non-CT, USA Domestic customers only)

The RA number for your housing is the serial number of the X-ray tube that you purchased. You do not need to submit any form to request an RA. Your paperwork is included with your new tube.
