Warranty and Housing Returns
To return a tube for warranty evaluation or repair, please select a warranty return type below. Fill out all the required fields, and please also provide any other information you have regarding the tube being returned. Tubes must be shipped prepaid, along with a completed Tube Service Report form describing the reason for return, operating conditions, installation and removal dates in order to receive warranty consideration.
